This year physicals will be a little different. If you are a 7th-11th grader (this year), and you do NOT answer yes to anything on the medical history form then you do NOT have to get a physical. If you answer yes on anything you have to get a physical.

ALL incoming 7th Graders will need to get a physical.

I will also have extra medical history forms in the girls coaches office (Gym). I will be up here tomorrow from 9:30-12:00. If you would like to come by and pick one up, email me at mneighbors@lindsayisd.org

If the link does not work for you, you can go to uiltexas.org. Under athletics you will click on Forms, then under Medical Forms you will see Athletic and Marching Band Pre-Participation Physical Evaluation form. That’s the one you will click on.
